- August 2024: Our GP service and our waitlist are now closed to new clients seeking gender affirming care. To access any of our other services, please use the intake form below. If you already have accessed Peer Navigation, Allied Health, or other YourCH services in the past, please send us an email with your name, last name, DOB, telephone number and postal address to [email protected].
- May 2023: If you are one of our valued clients who gets their hormone injections administered by our team while having your prescriptions issued by an external provider, please click here to access our form and bring us the information listed, so we can ensure your clinical safety.
Our program is a Victorian Government funded initiative that aims to increase body and gender affirming health services across Victoria. We work in partnership with Ballarat Community Health, Thorne Harbour Health and Austin Health. All clinics oversee their own client intake and operations.
We provide healthcare and gender affirmation services to trans, gender diverse and non-binary Victorians, people with sex variations, and anyone seeking endocrine assisted medical affirmation support.
In addition to our specialist Endocrinologist, GP and Nurse services, we also provide:
- Peer Navigation
- Referral to social and peer support groups
- Access to affordable Counselling, Physiotherapy, Dietetics, Podiatry and other allied health services.

Please fill out the registration form below and someone from our Peer Navigation team will contact you. Peers are here to help you understand your options for planning and accessing the types of care available across the State. Our Peer Navigation service is proudly funded the Victorian Department of Health, and it is free of charge.
As your first appointment with our service will be by phone with one of our Peer Navigators, we use an online registration form to save you time. Completing it takes less than 10 minutes and, once submitted, we will contact you shortly to book your appointment.
