Your Community Health is pleased to announce that from November 2024, Austin Health’s Gender Clinic (Endocrinology) will be moving to Your Community Health (YourCH).
All Endocrinologists are joining the YourCH team, so you will continue to receive care from the clinician you know. Please also note that, upon moving to YourCH, the Endocrinology Clinic will become a mixed billing clinic. This means that you will be charged an out-of-pocket fee for your appointment. This fee allows us to provide you with high quality care you know and expect. What We Do
We provide healthcare and gender affirmation services to transgender, gender diverse and non-binary Victorians, people with sex variations, and anyone seeking endocrine assisted medical affirmation support.
You can make an appointment with us if you want to start hormone therapy or are considering medical transition. Your needs may also be met by a GP, and our team help you understand what type of clinician you could see for your unique needs.
Our clinic staff are experts in transgender health. Many of them also work as researchers and educators to improve the health and wellbeing of trans people.
We work closely with the trans community, GPs, community organisations and researchers. We work in partnership with Ballarat Community Health, Thorne Harbour Healthand Austin Health.
Our care is based on the informed consent model developed by the Equinox Gender Diverse Health Centre.
What to expect
You will need a valid referral from a GP to access this service. Please send a copy of your GP referral through to endocrinology@yourch.org,au.
Please note that referrals last 12 months from the date of your first appointment and will be required to provide an updated referral once this expires.
Once we receive your referral, it will be triaged and one of our staff will get in contact with you to make an appointment.
If you are a previous patient of Austin Health’s Gender Clinic (Endocrinology), you will be required to provide a new referral as this clinic has changed. Please note that to support the Endocrinologist with continuity of your care, we request you complete the following Medical Transfer Form to enable us to access your previous medical notes and results from Austin Health specifically in relation to the Endocrinology Clinic. What to bring
- When you come for your appointment at YourCH, you need to bring:
- Medicare card
- Health care card/concession card (if applicable)
- Adverse drug alert card (if you have one)
- X-ray films, scans, ultrasounds, pathology or any other test results or reports
- List of medicines you are currently taking (or the boxes) including herbal supplements and vitamins
- Glasses, hearing aid, walking frame (if applicable)
- Your blood glucose meter and log book (if applicable)
Fees
Our service will continue to offer bulk billing medical services to clients with a valid medicare card who are:
- Aboriginal and/or Torres Strait Islander persons
- Refugees and Asylum Seekers.

Please note: Fees must be paid on the day of your appointment. Any outstanding debt may result in not being able to book a follow up appointment until the debt is paid.
You are only eligible for a Medicare rebate for Endocrinology services if you have a valid referral.
If you do not have a valid referral on the day, you will need to pay the full fee and will not be eligible for a Medicare Rebate.
