Your Community Health is a provider of Commonwealth subsidised National Disability Insurance Scheme (NDIS) services and is therefore required to comply with the NDIS Practice Standards.
We are assessed against these Standards to ensure we deliver high-quality NDIS supports and services that promote choice, control and dignity.
As part of the assessment process, we are required to provide evidence of compliance with and performance against these Standards. On 28-29 October 2024, we will also undergo a formal audit via QIP against the NDIS Practice Standards. All YourCH clients of NDIS services are automatically enrolled into the audit process and may be contacted to participate in interviews and/or have their files, records or care plans reviewed to ensure we are compliant with the NDIS Practice Standards. You can opt out of the audit process by telling one of our clinicians/service providers that you prefer not to participate, or contacting us on quality@yourch.org.au.
