We offer a variety of aged care services and supports to help you feel your best as you get older.
If you are 65 years and older, or are an Aboriginal and/or Torres Strait Islander person who is 50 years and older, you can access aged care services via My Aged Care.
Our aged care services are offered either at our health centres or in your home, if appropriate, and include:
- Counselling
- Podiatry
- Physiotherapy
- Physiotherapy Exercise Groups
- Nutrition and Dietetics
- Occupational Therapy
- Social Support Groups.

Eligibility
To use our aged care services and supports, you will need a referral via My Aged Care. Aged Care services are available to people 65 years and older, and Aboriginal and/or Torres Strait Islander people 50 years and older.
If you are under 65 years old, you may still be eligible for aged care services. We encourage you to contact My Aged Care online, on the phone (1800 200 422) or in person to confirm.

When you contact My Aged Care, you can let them know that you wish to use Your Community Health services
When you already have a Home Care Package: If you are already registered with My Aged Care and on a Home Care Package, you can complete our online referral form or download the referral form and email it to us at referrals@yourch.org.au.

Help with My Aged Care
- If you need extra help to access aged care services, you could be eligible for support through the Care Finders program.
- For translating and interpreting services (TIS National), call 131 450 and ask for 1800 200 422.
- For Aboriginal and Torres Strait Islander interpreting services, call My Aged Care on 1800 200 422 and ask for an interpreter. There is a list of available languages on the Accessible for All page.
- For Sign Language Interpreting Services, contact Deaf Connect on 1300 773 803 or interpreting@deafconnect.org.au in advance to ensure an interpreter is available.

My Aged Care is your starting point for accessing Australian Government-funded aged care services.
They provide the information and support you need to understand, access and navigate the aged care system. You can access My Aged Care online, on the phone or in person.
My Aged Care provides:
- information on the different types of aged care services available
- an assessment of your needs to identify eligibility and the right type of care for you
- referrals and support to help you find service providers that meet your needs
- information on what you might need to pay towards the cost of your care
